Unfortunately, he did not. If you </span><em>are</em><span> a man who has sex wh men, you won’t be rmed of your particular risk om sexual exposure — which is a problem, bee </span><a href=" rel>CDC data as of late July showed that 99% of US monkeypox s had been men</a><span>, and 94% of those men wh data provid to the CDC reported “sexual or close timate ntact” wh another man wh the 3 weeks prr to the start of symptoms.</span></p><p><span>And even if you — still a gay man, this hypothetil — read between the l of Menza’s weirdly pdish mg about what nstut “prolonged sk-to-sk ntact” (“perhaps massagg… or huggg… or perhaps cuddlg…”) and fer that </span><em>sex</em><span> might be one of the major risks here, you’re still liable to be improperly reassured by Menza’s overemphasis on the importance of ntact wh sor and sbs.</span></p><p class="button-wrapper" data-attrs="{"url":"" data-ponent-name="ButtonCreateButton"><a class="button primary" href=" rel><span>Share</span></a></p><p><span>Most people, I thk, stctually avoid extend ntact wh other people’s sor and sbs whout havg to be told to do that by a public health profsnal.</span><div class="footnote-hoverrd-target"><a class="footnote-anchor" data-ponent-name="FootnoteAnchorToDOM" id="footnote-anchor-1-68079182" href=" rel>1</a></div><span> But if you’re led to believe that ntact wh sor and sbs is sential to spreadg monkeypox, then you might fer that ’s low-risk to have sex wh someone who has clear sk. And until very recently, </span><a href=" rel>the CDC was reassurg the public that ’s impossible to spread monkeypox whout symptoms</a><span>. But as the NYC Department of Health has been rrectly warng gay men, </span><em>we don’t know if that’s te</em><span> — and the manner which this outbreak has propagated self would ankly make more sense if there is a signifint gree of non-symptomatic spread. </span></p><p>Menza did eventually get around to talkg about gay men, sort of, when prompted by his terviewer (who, of urse, went directly to the issue of “stigma” before anyone voted any time to the qutn of whether gay men need any particular rmatn about risk rctn):</p><blockquote><p><em><strong>Chávez:</strong><span> Right now my unrstandg is that many of the s Oregon have been found cisgenr men who have sex wh men. What we’ve been tryg to do as bt as we n is stick wh what we know. In the Uned Stat, we know that </span><strong>people assigned male at birth who have sex wh men and people assigned female at birth</strong><span>, cludg at least one pregnant person, have been affected by hMPXV Oregon. We know that cisgenr men and nonbary people are affected by hMPXV. While most intify as gay or queer and report close ntact wh people assigned male at birth, we have s that also intify as straight and bisexual and report close ntact wh people assigned female at birth.</span><strong> (emphasis add)</strong></em></p></blockquote><p><span>It is simple enough to say that monkeypox is spreadg pecially among </span><em>men who have sex wh men</em><span>. If you also say that there is elevated risk for </span><em>trans and nonbary people who have sex wh men</em><span>, you’ve vered the whole universe of elevated sexual risk for monkeypox a clear and unrstandable way. Instead, Menza starts by talkg about, for some reason, “people assigned male at birth who have sex wh men and people assigned female at birth” — that is, to a first approximatn, gay men </span><em>pl women</em><span>, which is not anywhere close to an accurate scriptn of the high-risk group.</span></p><p><span>It is unacceptable, if you have Dr. Menza’s job, to speak woke word salads like this. Menza is speakg on a broadst news program. It is his </span><em>job</em><span> to munite clear terms to the broad public. They rely on “muny partners” to liver that secret msage. </span><em>Well, first of all, what about gay men who listen to Oregon Public Broadstg? </em><span>I promise you there are gays listeng to public rad. And if they hear this terview, they’re gog to e away ls rmed and ls equipped to avoid monkeypox than they ought to be.</span></p><p class="button-wrapper" data-attrs="{"url":"" data-ponent-name="ButtonCreateButton"><a class="button primary" href=" rel><span>Share</span></a></p><p><span>Another problem wh this secret-msage strategy is that rmatn about who’s a high-risk group isn’t relevant only to people the high-risk group. It’s relevant to people </span><em>not</em><span> the group, bee helps them figure out their risk isn’t high. And this go to the part of the monkeypox msagg nonsense that </span><em>isn’t</em><span> polil rrectns — ’s nrotic public health officials beg veterately pable of tellg anyone their risk is low due to mographics.</span></p><p>But the biggt problem wh this msagg strategy is simply that is dishont. </p><p>Menza knows somethg — that this outbreak is happeng almost entirely through gay sex — but this terview, he’s hidg the ball about that bee he don’t tst the broad public wh that rmatn. He’s ncerned that knowledge of this fact will e people to stigmatize gay men.</p><p><span>I first of all thk is crazy — if you want to </span><em>avoid</em><span> stigmatizg gay men — to duce straight people to be unnecsarily aaid of acquirg, through sual ntact, a disease that is spreadg mostly among gay men through sex. But I would also note that we have jt gone through a multi-year procs which the public teem for the public health apparat has dimmed nsirably. People have noticed that public health statements were not always on the level, and that mak them ls cled to tst those statements. (Remember the CDC tellg people March 2020 not to wear masks? And then not acknowledgg airborne COVID spread until October 2020? Remember thoands of ‘experts’ signg that June 2020 letter sayg was sudnly okay to go out public so long as you were prottg for the right polil ?)</span></p><p>And public health officials, stead of swallowg their pri and tryg to rega public tst, have been credibly arrogant, ntug to view themselv as the judg of what rmatn the public n and n’t be tsted wh, as they try to supply and whhold that rmatn the way they thk will bt shape public behavr toward their own iologil ends.</p><p>Frankly, I feel a great al of spair about this suatn. We do need public health experts. Their work, when done well, is important. But the field is full of leftist weirdos who let iology get the way of facts and who n’t help themselv om puttg their thumbs on the sle to promote their preferred iologil out. As a rult, many of them are pable of dog their jobs properly, pecially when the job scriptn entails behavg a manner that might engenr the tst of the broad public. And I don’t know what to do about that.</p><div><hr /></div><p>Speakg of tst officials, FDA Commissner Robert Califf has ma a pretty “whoa, if te” announcement about monkeypox vacc: That we’ve found a way to oculate people wh jt one-fifth the normal vacce dose amount. The FDA says jectg the JYNNEOS vacce trarmally (that is, wh the sk) duc a much stronger antibody rponse than the ual subcutaneo jectn, which turn mak possible to greatly shrk the vacce dose, so we n get 5 or 6 shots out of what is supposed to be a one-dose vial.</p><p>Or at least, the FDA thks will work that way.</p><p><span>I am genuely unsure what to make of this cisn, and </span><a href=" rel>relevant experts are divid on </a><span>. It do seem clear that the smaller trarmal jectn do jt as well at </span><em>ducg the productn of antibodi</em><span> as the larger subcutaneo jectn. What’s ls clear is whether that means ’s siarly equivalent for producg immuny — immune rpons are plex and, as many of learned durg COVID, don’t always simply rrpond to antibody ter levels. It’s also unclear how quickly lol health partments n sle the new vacce strategy, which will be more plited to admister and require retrag the profsnals who provi the jectns.</span></p><p><span>The problem is we don’t have any </span><em>available</em><span> optns that are known to be better than the one the FDA is advancg, sce we don’t have enough vacce to immunize the relevant populatn through the ual admistratn method, and we likely won’t until well to 2023.</span></p><p><a href=" rel>As I’ve wrten before</a><span>, several large health partments are currently alg wh the vacce shortage by layg the send dose of the two-dose vacce regimen. As wh the swch to an trarmal jectn, ’s currently unknown exactly how much that impacts effectivens. Another optn is to e an olr smallpox vacce that we have much more ample supply, ACAM2000, but that vacce has an unappealg si-effect profile and also (unlike JYNNEOS) nsists of a replitg live vis that n be spread through ntact wh the jectn se.</span></p><p>In the difficult circumstanc, I am pleased to see the FDA gettg creative a suatn where creativy was need. But we wouldn’t be this posn if the FDA and other agenci had ma better cisns months and years ago.
